Before 1994, Formula 1 had a quiet understanding with death. It was treated as an occupational hazard: tragic, but somehow inevitable. The sport mourned, adjusted marginally and moved on. Speed was the product. Risk was the price.​

When Ayrton Senna crashed fatally at the 1994 San Marino Grand Prix, the illusion of acceptable risk shattered. The sport could no longer pretend that fatalities were random acts of God rather than the predictable output of a system not designed carefully enough.
